Please complete all the required fields in order for the student at your institution to apply for an Educational Assistance Payment. Once you have completed this process, please click “Finish and Print" to download a printable PDF version of the Enrolment Confirmation Form. Once you have downloaded and printed the PDF, please sign and affix the institution stamp or seal where specified and forward the completed form to Children's Education Funds Inc. by mail, fax or a scanned copy by e-mail so that it is received no later than
